Membership Certificate Request Form
If you are not using the online invitation system, please be sure to use the 2021 Membership and Merchandise order form for all orders -- even if you are just ordering one honor cord. To use the online invitation system to process memberships, please contact the national office at .
Medallions and honor cords have are $17 each or $30 for a combo pack. The lapel pin no longer comes as part of the $35 membership fee. If your students would like a pin the fee is $1.00 extra.
Completing the form:
Worksheet One is the actual order form. Advisors should complete the top portion of the form with current chapter information, including the scheduled date of initiation. The form also asks for the date the Chapter Advisor first took on the role of advisor on campus. The middle portion of the form allows you to enter a number for each item ordered. The form tabulates the cost for membership and merchandise.
Worksheet Two (look for tab at bottom labeled “new members”) is the location to enter names, email addresses, and graduation term information.
Name the file with your institution, chapter name, and term (ex: University of Texas Austin_Alpha_Spring 2021 order).

If you wish to pay by credit card, please email the national office with your order and they will send you a digital invoice, which provides access to a secure online payment portal. If you wish to pay by check, please submit the payment directly to the national office at Pi Sigma Alpha, 1527 New Hampshire Ave., NW, Washington, DC 20036.
Delivery upon payment:
As certificate requests come to the national office we process them in order of receipt. We will print and package certificates and merchandise but will NOT mail them until payment has been received. All mail is shipped USPS Priority Mail. Please submit information and payment early so your certificates arrive on time!
Chapter Annual Report
Chapters are required to submit an annual report to the national office which identifies key student officers and chapter advisor, and lists the chapter’s activities for the past year. The information in this report is used to select the winners of the Best Chapter Awards, so chapters should take care to make them as complete as possible.

Chapter Activity Grant Report Form
Chapter that were recipients of a Chapter Activity Grant are required to submit a report on expenditures by June 15. The report may be submitted by email or postal mail.
